We are PROFESSIONAL
- PEAK’s services are performed by PCA’s, CNA’s, LPN’s or RN’s depending on level of care
- All care is under the direction of Peak’s supervising Registered Nurse
- A Registered Nurse is available 24 hours for emergencies
- 24 hour on call availability by Peak Staff for emergency needs
- All care providers are Peak employees protecting patients from any responsibility or liability
- Peak care providers read, write and speak English
- Have passed extensive background checks and have current certifications
- Are subject to random drug testing, finger printed, and are individually bonded and insured
- Are CPR certified and undergo annual TB testing and physical examinations
- Attend ongoing education and training sessions
- Meets State of Illinois licensing requirements for background checks, supervision, documentation, training and consumer education

- Consultation visit determines patient needs, abilities, restrictions, special circumstances, care goals and starting schedule
- Registered Nurse’s medical assessment within 24 hours of admission
- Nurse/Physician Care Plan of treatment that determines care duties and expected outcomes
- Regular follow-up reporting to Physicians with Care plan updated as needed
